Germantown Death Records Lookup

Germantown death records are processed through Montgomery County, which operates its vital records office at the Dennis Avenue Health Center in Silver Spring. Germantown is one of the largest unincorporated communities in Maryland, but it has no city government or local vital records office. Residents need to contact Montgomery County Health Department or the Maryland Division of Vital Records to get certified death certificates. This page explains the exact process, office hours, fees, and what to do if the record you need is from before 2015.

Where Germantown Residents Go for Death Certificates

The Montgomery County Vital Records Office is at the Dennis Avenue Health Center, 2000 Dennis Avenue, Suite 27, Silver Spring, MD 20902. For Germantown residents, that is about 12 miles away. There is no separate Germantown vital records office -- all Montgomery County residents use the same location in Silver Spring.

For death-related vital records, call 240-777-1757. For birth-related matters, the number is 240-773-1207. The office offers bilingual services in English and Spanish.

One important procedural note: when you arrive, check in at the lobby table before going to the Vital Records office. Do not go directly to the office window without checking in first at the lobby. This is required, and skipping it can delay your visit.

Montgomery County Vital Records: Hours and Schedule

The schedule at the Dennis Avenue Health Center varies by day. On Monday, Wednesday, and Friday, the office accepts walk-in visitors from 8:00 to 11:00 a.m. Appointments are available those same days from 1:00 to 3:00 p.m. On Tuesday and Thursday, the office is appointment-only, with slots available from 8:00 to 11:30 a.m. and from 1:00 to 3:00 p.m. The office closes for lunch from 11:30 a.m. to 1:00 p.m. on all days.

Walk-in service is first-come, first-served during the morning windows on Monday, Wednesday, and Friday. Capacity is limited. If you arrive late in the walk-in window, you may not be served that day. To avoid the uncertainty, scheduling an afternoon appointment is more reliable.

Same-day service is not guaranteed. Montgomery County implemented a new registration system recently, and that system has been causing processing delays. The county advises that some requests may not be completed the same day even with an in-person visit. Plan ahead if you have a deadline.

Montgomery County Death Certificate Fees

Fees changed effective September 2, 2025. The current fee for a first certified death certificate is $18. Each additional copy ordered at the same time costs $20. Note that the additional copy fee is higher than the first copy fee -- this is not a typo. The county sets its own fee schedule separate from the state DVR.

The office accepts cash, credit cards, Apple Pay, and checks or money orders payable to "Montgomery County, MD." Personal checks are accepted. If you are paying by check, make it out exactly as stated -- "Montgomery County, MD" -- to avoid delays.

These county fees apply when you obtain a certificate in person at the Dennis Avenue office. If you order through the Maryland Division of Vital Records by mail, the state fee of $24 for the first copy applies instead. VitalChek online orders use the state fee plus a service charge.

The Montgomery County office issues death certificates for deaths that occurred in Maryland on or after January 1, 2015, and that are registered in the current system. Deaths before January 1, 2015 are not available at the county office. For those older records, you need to contact the Maryland Division of Vital Records or the Maryland State Archives.

The Maryland Division of Vital Records handles all pre-2015 records by mail or appointment. Their address is 6764-B Reisterstown Road, Baltimore, MD 21215, with a mailing address of P.O. Box 68760, Baltimore, MD 21215-0036. The phone is 410-764-3038. Mail orders take four to six weeks. In-person visits require an appointment.

For deaths from 1973 through 2014, the Maryland Death Certificate Index at the State Archives is searchable online. It gives you enough identifying information to confirm a record exists and place a formal request. For records that are hard to find, the Archives has a dedicated guide explaining why gaps occur and what alternatives are available.

What You Need to Bring to the Montgomery County Office

To get a certified death certificate in person at the Dennis Avenue office, you must provide a valid government-issued photo ID. The ID must have both an issue date and an expiration date. A driver's license, state ID card, or passport all work. Expired IDs will not be accepted.

You also need to establish your relationship to the deceased. Accepted entitlement documents include a birth certificate that shows your relationship, a marriage certificate, or an obituary that names you as a surviving relative. Maryland does not issue certified death certificates to people who cannot document their eligibility.

If you are representing a legal estate, bring documentation of your authority to act -- a letter of administration or similar court document will help establish your right to the record.
